  Credentialing By Laws

*If you are already credentialed at one of our facilities, please contact Chrissy Hayes on credentialing@montserrat.com.au and arrangements can be made to access your paperwork.

In order to process your application in the quickest possible manner, we request that you provide the following:

• Copy of your current medical registration
• Copy of your current medical indemnity insurance showing specialty covered and expiry date
• Copy of your CV

Review of Credentialing Application

• Upon reviewing your completed application, the application will be reviewed by the CEO or Chairman of Montserrat Day Hospitals
   
• At this point temporary accreditation may be granted for a short period of time
   
• Supported applications are then referred to the Credentialing Committee (held 3 times per year) for consideration
   
• Upon endorsement by the Credentialing Committee, the doctor's names and details are then referred to the Montserrat Board for ratification
   
• A letter confirming Accreditation at Montserrat Day Hospital/s (Spring Hill, Indooroopilly, Gaythorne or North Lakes) will then be forwarded to the doctor

North Lakes Day Hospital Credentialing


Specialists interested in operating at North Lakes Day Hospital are advised to provide their application as soon as possible in order that there are no delays upon opening in late 2011. Please follow the above process with this application.
