Patient Check List

Before the Day of your Procedure

• Check time, date and place of procedure
• Arrange transport home. Do not drive
• Ensure that you have read and followed the instructions relevant to your procedure

Day of Procedure

• Bring medications that would normally be due to take
• Wear comfortable, loose clothing
• Leave jewellery and valuables at home
• Do not wear contact lenses (if they are the type that requires removal at night time)
• Bring your health insurance details and payment to finalise your account

After procedure

• Do not drive for the time specified un your procedure leaflet
• Read discharge instructions given to you at the time of discharge
